Telehealth Architecture: WebRTC, Secure Audio/Video, and Low-Latency Clinical Care

By Houston Software Engineering · Published · Updated · 8 min read

Video is only one part of the care workflow

A telehealth product must coordinate identity, consent, scheduling, reminders, intake, the visit, documentation, follow-up and billing. Video is important, but a reliable connection does not compensate for a confusing check-in or missing clinical context.

Map what happens before, during and after the session. Decide which system owns the appointment, how the patient and provider are matched, and where the resulting record belongs.

Treat the session as a secure, expiring context

Use strong identity verification, scoped session tokens, appropriate role checks and a clear expiration model. A patient should only enter the intended visit, and a provider should be able to confirm who is present before sensitive information is discussed.

Be explicit about what is and is not recorded. If the product handles notes, documents, messages or recordings, each has its own access, retention and vendor considerations beyond the video connection itself.

Design for failure and accessibility

Calls drop, cameras fail, browsers deny permissions and networks change. Give both sides a clear recovery path: reconnect, switch device, use audio, send a message or reschedule without losing the relevant context.

Support keyboard navigation, readable instructions, captions or other accommodations appropriate to the service. Patients should know what is happening and what to do next even when the technology is not behaving perfectly.

Integrate the care record and measure the real outcome

Connect scheduling, patient identity, forms, documentation and billing with the systems that own those records. Avoid making staff copy the same information into the EHR after every visit.

Measure completed visits, failed check-ins, drop-offs, reschedules and support requests. Those signals reveal whether the product is improving access and clinical operations, not just whether the video SDK connected.
